- Recognizing The possibility of physical injury associated with soccer and in consideration for Montoursville AYSO 690 accepting the registrant for its Adult Soccer League, I hereby release, discharge and/or otherwise indemnify the Montoursville AYSO 690 and their employees/volunteers, including the owner of fields, as well as the founders of the Keystone Adult Soccer League, their volunteers and referees, against any claim by or on behalf of the registrant as a result of the registrant’s participation in the League and/or being transported to or from the same, which transportation I hereby authorize.

- Hereby give my consent to have an athletic trainer and/or doctor of medicine or dentistry provide me with medical assistance and/or treatment and agree to be responsible financially for the reasonable cost of each assistance and/or treatment should an injury or illness occur.

- I understand that the members of the Keystone Adult Soccer League are responsible for any damage to the fields or equipment that may occur. I will be directly responsible for any damage that I cause to any property owned by Montoursville AYSO 690 or the Keystone Adult Soccer League, it’s representatives, volunteers, or referees.

- By submitting the above form and playing in the Keystone Adult Soccer League, you give permission for Keystone Adult Soccer League, its organizers, volunteers, assignees, employees, and other KASL permissible parties, the authority to use your recordings, quotes, photographs, movies or video tapes, and likeness for the purposes of promoting the Keystone Adult Soccer League, Montoursville AYSO 690 and their affiliates.
